CKD Stewardship is the opportunistic early identification of patients with mid to late-stage CKD while they are in hospital for an unrelated problem.

We use the eMR (Powerchart or EPIC) to filter all adult patients admitted to hospital by eGFR. The files of patients with an eGFR <45ml/min/1.73m2 on admission are then reviewed remotely. Adult patients <80 years, not already known to a nephrologist, who are not in palliative care, do not have dementia or a terminal illness, are then included for CKD-Stewardship.

Our kidney nurses speak with the patient about CKD, inform them of the likely diagnosis and ask them to see their GP for a kidney health check.
Our kidney doctors write a note in powerchart/EPIC informing the home team of the diagnosis.
We send a one page CKD discharge letter to the GP with Guideline Directed Medical Therapy (GDMT) advice based on the patient's eGFR.
